I hereby apply for club membership of Penicuik Harriers and agree to abide by the club constitution and that of Scottish Athletics.

I give permission for my details to be seen by committee members and coaches and to be stored on the computer.

I will inform coaches of any relevant health problems, at the beginning of each club training session.
